Job summary

Bedford Care Center of Petal in Mississippi is seeking a dedicated Social Services professional to identify and address residents’ social, emotional, and psychological needs and assist with discharge planning. Strong communication skills, MS Office proficiency, and a valid Mississippi driver’s license are required.

Responsibilities include care planning, interdisciplinary CQI participation, documentation, and family counseling.

Qualifications

• Bachelor’s or Master’s degree in Social Work or related field.
• Two years Social Services experience in Long-Term Care.
• Proficient in Microsoft Office; valid Mississippi Driver’s License; pass drug screens and background check.

Responsibilities

• Develops Social History, Social Assessment, Care Plan to identify problems, needs, set realistic goals, and needs upon Admission, compliant with State and Federal Regulations.
• Participates in interdisciplinary Care Plan Meetings and CQI Program.
• Documents Progress Notes for Resident’s Care Plan when necessary within Company Policy time frames.
• Assists Residents and family members with discharge planning and discharge process.
• Promotes communication assistance and support for family members and Residents; provides assistance with TDD phones and/or other communication assistance for impaired Residents.
• Attends public relations activities as requested by the Administrator and assists with community education regarding aging, Facility services and other related information.
• Participates in pre-admission screening of potential Residents and with providing information to Residents and families during the admissions process.
• Orients Residents to long-term care environment, Facility policies and Facility’s staff.
• Interviews, orientates, trains, supervises and evaluates Social Services personnel; makes work assignments, sets goals and priorities; keeps associates informed of all changes.
• Manages ordering of supplies; maintains records for budget and supplies.
• Attends Family Council and Resident Council Meetings; counsels with families.

Job Summary

Responsible for identifying and providing each Resident’s social, emotional and psychological needs for the continuing development of the Residents’ full potential; assists with Discharge Planning for Transfer and Discharge; must exhibit a sincere desire to work with the elderly and communicate courteously with Employees, Residents and others.

Requirements / Qualifications

Prefer Bachelor’s Degree or Masters Degree in Social Work, or two years Social Services experience in Long-Term Care; must have basic computer skills in Microsoft Office, excellent oral and written communications skills and have a valid Mississippi Driver’s License; must pass drug screens and have a satisfactory Criminal History Background Check.

• Ensures Residents’ personal property lists are updated at least annually


• Cooperates with and assists the Activity Department with Residents’ activities


• Ensures compliance with and completion of the MDS, CAT’s and CAA’s within pre-set time frame and criteria.
