The role emphasizes advocacy, care coordination, and collaboration with an interdisciplinary team to ensure residents and families receive comprehensive support throughout their healthcare journey.

Qualifications

• Bachelor's degree in social work or related field preferred.
• Colorado social work licensure or eligibility preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.

Responsibilities

• Oversee and direct social services operations within the facility.
• Conduct psychosocial assessments and maintain required documentation.
• Participate in care planning and provide input for MDS assessments.
• Advocate for residents' emotional, social, and mental well-being.
• Collaborate with the interdisciplinary team during care conferences and care planning.
• Assist residents and families with discharge planning, transfers, and accessing community resources.
• Build positive relationships with residents, families, and community partners.
• Support smooth transitions of care and discharge planning.
• Maintain compliance with applicable state and federal regulations.
• Help support resident engagement and quality-of-life initiatives.
• Provide guidance and leadership within the social services department.

Your Role

As the Social Services Director, you will play an important role in supporting the emotional, social, and overall well‑well-being of our residents.

Who We're Looking For

• Bachelor's degree in social work or a related field preferred
• Current Colorado social work licensure or eligibility preferred
• Previous experience in skilled nursing, rehabilitation, or long‑term care strongly preferred
• Knowledge of MDS, care planning, and regulatory requirements preferred
• Strong communication, organizational, and leadership skills
• Compassionate and committed to resident advocacy and person‑centered care
• Ability to work collaboratively with residents, families, and an interdisciplinary healthcare team
