Job summary

Pine Ridge Rehabilitation & Healthcare Center in Pagosa Springs, Colorado is seeking a compassionate Social Services Director to lead our social services department and support resident-centered care within our skilled nursing and rehabilitation community.

You'll oversee psychosocial assessments, participate in care planning and MDS input, advocate for residents' emotional and social well-being, and collaborate with the interdisciplinary team to ensure smooth transitions and regulatory

Qualifications

• Bachelor's degree in social work or related field required.
• Colorado social work licensure or eligibility preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Knowledge of MDS and care planning helpful.

Responsibilities

• Oversee and direct social services operations within the facility.
• Conduct psychosocial assessments and document findings.
• Participate in care planning and provide input for MDS assessments.
• Advocate for residents' emotional, social, and mental well-being.
