Job summary

Sequoyah Pointe Living Center is seeking an experienced Social Services Director to join our leadership team. You will work with residents and families to ensure compassionate care and meaningful engagement in our long-term care community.

Responsibilities include coordinating admissions and discharges, completing assessments, and participating in care plan meetings, while advocating for residents' rights and coordinating with staff and outside providers.

Qualifications

• Long-term care experience required.
• Social Services experience preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to work effectively with residents, families, staff and outside providers.
• Knowledge of long-term care regulations and resident rights.
• PCC experience is a plus.
