Job summary

Symphony Applewood is seeking a compassionate Social Service Director to support our short-term rehab and long-term care guests. This role provides psychosocial support, discharge planning, advocacy, and care coordination for residents and families, collaborating with the interdisciplinary team.

The ideal candidate holds a Bachelor's Degree, 2–3 years of long-term care experience, and is knowledgeable about federal and state regulations governing care facilities.

Qualifications

• Bachelor’s degree required.
• 2–3 years of long-term care and rehab experience preferred.
• Knowledge of federal and state long-term care regulations.

Responsibilities

• Conduct psychosocial and cognitive assessments.
• Develop and maintain resident care plans.
• Assist with discharge planning and transitions of care.
• Provide emotional support and advocacy for residents and families.
• Collaborate with nursing, physicians, and interdisciplinary team members.
• Maintain accurate documentation and participate in care conferences.
• Support end-of-life, hospice, and palliative care discussions.
