Job summary

Symphony Applewood seeks a compassionate Social Service Director to lead psychosocial support, discharge planning, and care coordination for short-term rehab and long-term care guests. You will collaborate with residents, families, and the interdisciplinary team to ensure quality outcomes.

The role requires a Bachelor’s degree and 2–3 years in long-term care or rehab, with strong communication, empathy, and problem-solving skills.

Qualifications

• Bachelor’s Degree required.
• Minimum 2–3 years of experience in long-term care and short-term rehabilitation preferred.
• Strong conflict resolution and problem-solving skills.
• Excellent communication and interpersonal abilities.
• Knowledge of federal and state regulations related to long-term care.

Responsibilities

• Conduct psychosocial and cognitive assessments.
• Develop and maintain resident care plans.
• Assist with discharge planning and transitions of care.
• Provide emotional support and advocacy for residents and families.
• Collaborate with nursing, physicians, and interdisciplinary team members.
• Maintain accurate documentation and participate in care conferences.
• Support end-of-life, hospice, and palliative care discussions.
