Flint Ridge Nursing & Rehabilitation in Newark, Ohio is seeking a Social Services Director to help residents adjust to life in our community, advocate for their needs, and support them throughout their stay. You will lead social services efforts and collaborate with the care team to ensure individualized plans.

Qualificações

• Education and credentials appropriate for the role and applicable requirements.
• Social services experience in a skilled nursing or long-term care setting preferred.
• Knowledge of care planning, discharge planning, and community resources.
• Strong communication, organization, and follow-through.

Responsabilidades

• Assess residents’ social and emotional needs and develop individualized care plans.
• Support residents and families through admissions, care planning, and discharge.
• Coordinate discharge arrangements, community resources, and referrals.
• Advocate for residents’ rights, preferences, and quality of life.
• Work closely with nursing, therapy, and other departments to address concerns.
• Maintain accurate, timely assessments and documentation.
• Participate in care conferences and help families understand available services.
