Job summary

CareCore Health is seeking a Social Services Director at our long-term care facility near North East Cleveland, Ohio. The role requires an LSW license and leadership to oversee social services for residents and families.

You will coordinate care planning, advocate for resident rights, and collaborate with staff to ensure high-quality psychosocial support within a compliant, compassionate environment.

Qualifications

• Licensed Social Worker (LSW) – Required
• Experience in a skilled nursing or long-term care setting strongly preferred
• Strong communication, organizational, and leadership skills
• Knowledge of community resources and elder care best practices
• A passion for working with seniors and making a meaningful impact

Responsibilities

• Assess resident social and emotional needs
• Develop and manage individualized care and discharge plans
• Serve as liaison among residents, families, medical staff, and external agencies
• Maintain accurate documentation, including MDS 3.0 and CAA’s
• Advocate for residents' rights and ethical care practices
• Coordinate psychological services and hospice referrals as needed
• Participate in interdisciplinary care planning and quality improvement efforts
• Ensure compliance with state and federal regulations regarding social services

Position Overview:
