Skilled Healthcare Facility in Mentor is seeking a Licensed Social Worker (LSW) to lead social services at our long-term care facility near North East Cleveland. You will oversee care planning and coordinate with residents, families, and staff.

The role emphasizes compassionate resident advocacy, strong leadership, and compliant documentation, including MDS 3.0, while working within a supportive team environment committed to senior care.

Qualifications

• Must hold a Licensed Social Worker (LSW) license.
• Experience in skilled nursing or long-term care settings preferred.
• Strong communication, organizational, and leadership skills.
• Knowledge of community resources and elder care best practices.
• Passion for working with seniors and making a meaningful impact.

Responsibilities

• Assess and address the social and emotional needs of residents
• Develop and manage individualized care and discharge plans
• Serve as liaison among residents, families, medical staff, and external agencies
• Maintain accurate documentation, including MDS 3.0 and CAA’s
• Advocate for residents' rights and support ethical care practices
• Coordinate psychological services and hospice referrals as needed
• Participate in interdisciplinary care planning and quality improvement efforts
• Ensure compliance with state and federal regulations regarding social services
