Job summary

CareCore Health in the Cincinnati area is seeking an experienced Social Services Director to lead the facility's social services program, coordinating psychosocial needs, care planning, discharge planning, and resident advocacy for residents and families.

The role requires strong communication, documentation, and collaboration with Nursing, Admissions, Therapy, and other departments to ensure resident rights and high-quality care across changes in condition.

Qualifications

• Long-term care/skilled nursing experience required.
• Social Services experience in a healthcare setting required.
• LSW preferred but not required.
• PCC experience preferred.
• Strong knowledge of resident rights, discharge planning, care planning, and psychosocial needs.
• Excellent communication, organization, and documentation skills.
• Ability to work effectively with residents, families, and an interdisciplinary team.

Job description

A skilled nursing facility in the Cincinnati area is seeking an experienced Social Services Director to join its leadership team. This position plays an important role in supporting residents and families throughout their stay while coordinating psychosocial needs, care planning, discharge planning, and resident advocacy.

Responsibilities
