Job summary

Journey is seeking a Social Services Director in the Frederick, MD area to oversee development, implementation, and evaluation of the Social Services Department for residents’ well-being. You will lead a team, ensure regulatory compliance, and coordinate across care plans and community transitions.

The role emphasizes collaboration with the home office and a focus on high-quality outcomes, continuous improvement, and resident-centered care within a supportive leadership structure.

Qualifications

• Bachelor’s degree in social work or another human services field.
• One year of supervised social work experience in a health care setting.
• License Social Worker License as required by state.

Responsibilities

• Identify the need for medically related social services and ensure services are provided per state and federal regulations.
• Complete or direct the social services component of the comprehensive assessment and contribute to care plans.
• Oversee preadmission services for inbound residents.
• Facilitate residents’ safe transition back into the community through discharge planning and follow-up services.
• Oversee establishment and modification of departmental QA procedures.

Responsibilities

• Identifying the need for medically related social services and ensuring that these services are provided in accordance with State and Federal regulations.
• The Social Services Director will complete and/or direct/delegate the completion of the social services component of the comprehensive assessment. The Social Services Director will also contribute to and/or direct/delegate contribution of social services goals and approaches to the care plan.
• Directing the Social Services department includes overseeing preadmission services for inbound residents
• The Social Services Director will facilitate residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
• The Social Services Director is responsible for overseeing the establishment of departmental QA procedures and modification of those procedures where appropriate.

Required Qualifications

All facilities must provide medically related social services to residents. It is not required that a qualified social worker necessarily provide all of these services, except as required by State law. The regulations do not require a Social Worker when a facility has equal to or less than 120 beds.

The Social Services Designee must possess:

• A minimum of a bachelor’s degree in social work or another human services field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology.
• One year of supervised social work experience in a health care setting working directly with individuals.
• License Social Worker License in required state.
