Job summary

Extended Care, LLC in Chicago, IL, seeks a dynamic Social Services Director to lead our Social Services team within an interdisciplinary care environment. You will monitor residents' psychosocial needs, develop ongoing care plans, and coordinate discharge planning with therapy, nursing, and other departments.

Requirements include a BSW or related field, 2–3 years in social services/discharge planning in long-term care, and strong communication and leadership skills.

Responsibilities

• Develop ongoing care plans based on residents’ needs, interests, and abilities identified in initial assessments.
• Participate in care plan meetings to ensure a comprehensive and coordinated approach.
• Complete required documentation upon resident admission, discharge, and transfer.
• Implement healthcare directives and identify end-of-life care opportunities.
• Assess residents' potential for discharge in collaboration with the interdisciplinary team.
• Plan and coordinate discharge services with residents, families, and community resources.
• Assist residents and families in navigating the discharge process and ensuring a smooth transition.

We are seeking a dedicated Social Services Director to lead our Social Services team. This department plays an integral role in our interdisciplinary team, working closely with therapy, nursing, and other departments to ensure high-quality care and smooth transitions for residents as they return home. The Social Services Director is responsible for monitoring residents' mental and psychosocial needs while providing services that enhance their overall well-being.

Social Services Director Requirements:
