Job summary

Signature HealthCARE in Danville, KY seeks a Social Services Director to identify and address residents' social, emotional and psychological needs, and to assist in discharge planning for a smooth transition from the facility. You will lead families through care decisions and coordinate with care teams to ensure person-centered planning.

Qualifications

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ years experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Overview
