Signature HealthCARE is seeking a Social Services Director in Danville, KY to identify and address residents’ social, emotional, and psychological needs and to assist in discharge planning during the stay. The role emphasizes professional, confidential support within a multi-state healthcare family.

The position requires a Bachelor’s or Master’s in Social Work or Psychology and 3+ years in social services, preferably in healthcare.

• 3+ years experience in Social Services, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and address residents’ social, emotional, and psychological needs.
• Complete timely documentation to comply with Facility, State, and Federal policies.
• Participate in resident care planning and discharge planning.
• Provide information and support for residents and families.
• Assist in planning for resident transitions and community reintegration.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.

How you Will make a difference

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

What you Need to make a Difference

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.
