Job summary

Signature HealthCARE is seeking a Social Services Director to identify and meet residents' social, emotional, and psychological needs during their stay and assist in discharge planning. You will support families, ensure compliant documentation, and participate in care and discharge planning with a compassionate, professional approach.

• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.

Overview

As the Social Services Director you will identify and provide for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
