Job summary

The Heights of Summerlin in Las Vegas, NV is hiring a Social Services Designee to support residents and families with placement, care plans, and transitions. You will work under the Social Services Director to ensure compassionate, patient-focused services.

Responsibilities include assisting with treatment wishes, participating in council meetings, supporting admissions, and discharge planning while maintaining confidentiality and adhering to regulations.

Qualifications

• High school diploma or equivalent is required.
• Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
• Knowledge of nursing practices, terminology, laws, regulations, and guidelines related to long-term care.

Responsibilities

• Assist residents and families in adjusting to placement, illness, and the plan of care.
• Help residents and families with their wishes regarding treatment and transitions/adjustments to the facility.
• Participate in resident and family council meetings as requested; support Admissions and discharge planning services.
