Social Services Designee Job Summary
The Social Services Designee participates as part of a multidisciplinary team to support the psychosocial well-being of each resident. This position serves as an advocate for residents and assists in addressing their psychological, emotional, and social needs.
The Social Services Designee works to help residents achieve and maintain their highest practicable level of physical, mental, and psychosocial well-being while following applicable federal, state, local, and facility regulations, guidelines, and standards.
Social Services Designee Responsibilities
- Monitor residents to identify unusual behaviors, changes, or concerns and report observations to appropriate professional staff.
- Provide mentally impaired or emotionally disturbed residents with routine physical, emotional, psychological, or rehabilitative support under the direction of nursing and medical staff.
- Work collaboratively as part of a multidisciplinary team that may include social workers, nursing staff, mental health professionals, and other healthcare team members.
- Organize, supervise, and encourage resident participation in social, educational, recreational, and therapeutic activities.
- Interview residents upon admission and gather appropriate information.
- Assist residents with bathing, dressing, grooming, and other personal care needs as appropriate.
- Participate in recreational activities with residents, including games, sports, television viewing, and other social activities.
- Help maintain resident restrictions and assigned areas according to care plans and facility guidelines.
- Accompany residents to and from medical and dental appointments, shopping trips, religious services, recreational events, and other approved activities.
- Help maintain clean, organized, and safe resident areas.
- Advocate for residents and promote their dignity, independence, emotional well-being, and quality of life.
- Communicate resident needs and concerns to the appropriate members of the care team.
- Perform other duties as assigned by the Supervisor or Administrator.
