The Laurels of Carson City is seeking a Social Services Designee / Discharge Planner to coordinate safe, timely discharges for residents. You will work with families, physicians, and the care team to address psychosocial needs and arrange post-acute services, ensuring regulatory compliance.

Qualifications

• High school diploma or GED.
• One or more years of discharge planning, case management, or care coordination in a healthcare setting preferred.
• Experience in skilled nursing, post-acute, or long-term care environments preferred.
• Working knowledge of discharge planning regulations, resident rights, and post-acute care resources.

Responsibilities

• Meet with residents or responsible party at admission and provide discharge follow-up.
• Collaborate with residents, families, physicians, nursing, therapy and case management to plan care.
• Arrange post-discharge services such as home health, equipment, hospice, transport, and resources.
• Attend care plan meetings, discharge planning meetings, and related sessions.
• Identify barriers to discharge and communicate needs to the Social Worker.
• Develop individualized discharge plans with the interdisciplinary team and family.
