Qualifications

• High school diploma or equivalent; two years of experience in a Social Worker role in healthcare.
• Preferred: Social Services Designee experience in Skilled Nursing Facility.
• Bachelor's degree in Social Work or similar and five years of related experience preferred.

Responsibilities

• Assist in preparing residents for their future and ensure their social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences.
