Qualifications

• Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
• Document progress in meeting the psychosocial needs of residents.
• Participate in the development of an interdisciplinary plan of care for each resident that identifies psychosocial needs and goals.

Responsibilities

• Coordinate the resident discharge planning process and arrange referrals for home care services.
• Provide therapeutic interventions to assist residents with transition and adjustment to the facility.
• Educate residents and families about their rights and available services, and refer to social service agencies when needed.
• Work with residents, families, staff to promote self-advocacy to improve quality of life and care.
• Provide groups for residents and families as appropriate.
