Minimum two years of Social Worker experience in healthcare is required; a bachelor’s in Social Work and five years of related experience are preferred, with growth opportunities and comprehensive benefits."

Qualifications

• High school diploma or equivalent and two years of experience in a Social Worker role, preferably in healthcare.
• Preference for prior Social Services Designee experience in a Skilled Nursing Facility and a bachelor’s degree in Social Work or related field with about five years of related experience.

Responsibilities

• Assist in preparing residents for their future and ensure social and emotional needs are met during their stay.
• Coordinate resident discharges and participate in resident care conferences to maintain high standards of care.
