Job summary

Monarch Healthcare Management is seeking a Social Services Designee to support residents and families through admissions and discharges. You will manage referrals, screenings, tours, approvals and documentation, while marketing services and building referral relationships.

As part of the interdisciplinary team, you will conduct social assessments, ensure compliance, and contribute to care planning and policy development to enhance residents' rights and well-being.

Qualifications

• Recognize and address psychosocial and emotional needs of residents.
• Experience in healthcare, skilled nursing facility or assisted living.
• Strong communication, organization, management and leadership skills.
• Able to perform essential functions with or without accommodation.
• Qualify for employment after a criminal background study per Minnesota DHS guidance.

Responsibilities

• Support residents and families through admission and discharge processes.
• Manage referrals, screenings, tours, approvals and documentation.
• Market facility services and build relationships with referral sources.
• Ensure regulatory compliance and maintain confidential, professional communication.
• Develop individualized care plans and participate in quality improvement efforts.
