Qualifications

• Must recognize and understand the psychosocial and emotional needs of residents.
• Experience in healthcare, skilled nursing facility or assisted living community
• Strong skills in communication, organization, management, and leadership.
• Able to perform essential functions of the position with or without accommodation.
• Must qualify for employment after a criminal background study (Minnesota DHS).

Responsibilities

• Support residents and families through admission and discharge processes.
• Manage referrals, screenings, tours, approvals, and documentation.
• Market facility services and build relationships with referral sources.
• Clarify legal documents and assist with advanced directives and financial aid.
• Conduct social assessments and required MDS/CAA documentation to develop care plans.
• Maintain records, ensure regulatory compliance, uphold confidentiality, and support quality improvement.
