Benedictine is seeking a Social Services Designee to coordinate the interdisciplinary care plan for residents from admission to discharge. You will advocate for residents and families and collaborate with healthcare staff to ensure prompt, thorough care.
Experience in hospitals or nursing homes is preferred. Strong English communication skills are essential as you build community resources and support for residents and their families.

Our Social Services Designee coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. You will act as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
