Responsibilities

• Coordinate the overall interdisciplinary plan of care for residents from admission to discharge.
• Serve as liaison between resident/family and healthcare personnel to ensure timely and effective care.
• Coordinate the psychosocial care plan for residents.
• Develop and maintain community resources for residents.
