Job summary

Reliant Care Management Company is seeking a Social Services Designee to plan and direct the Social Services Department within a long‑term care facility. You will assist with admissions, manage resident accounts, and coordinate with other departments to ensure comprehensive care.

The role emphasizes empathy, patience, and strong communication. Applicants should have experience in social work (2–5 years preferred) and familiarity with Medicaid/Medicare programs.

Qualifications

• Prefer 2-5 years of Social Work experience, ideally in Long Term Care setting.
• At least 1 year of supervised social work experience in health care setting working directly with individuals including those with mental challenges.
• Knowledge about Medicaid and Medicare programs is a plus.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department
• Assist with facility admissions
• Manage resident trust accounts and assist with resident shopping
• Schedule resident appointments and coordinate with other departments as needed
• Maintain accurate charts and documentation of resident care
• Engage, converse, and interact with residents to promote a positive environment
• Work primarily during daytime hours, with occasional evening and weekend visits as required
