Job summary

Monarch Springs Wellness & Rehabilitation in University City, MO, seeks a compassionate Social Services Designee to support residents’ emotional, social, and psychological well-being and to advocate for dignity and person-centered care.

You will conduct psychosocial assessments, aid in care planning, assist with transitions to long-term and memory care, and ensure accurate, compliant records while collaborating with an interdisciplinary team.

Qualifications

• Minimum bachelor's degree in Social Work or related Human Services field.
• Certified, licensed, or registered in the state of practice (required).
• At least 1 year of social work experience in healthcare or geriatric settings.

Responsibilities

• Support residents through psychosocial assessments and care planning.
• Assist with transitions to long-term and memory care for residents and families.
• Complete MDS-related documentation and maintain compliant records.
• Advocate for resident rights, dignity, and self-determination.
• Coordinate discharge planning and community resources.
• Collaborate with nursing, administration, and the interdisciplinary team.
• Assist with survey readiness and regulatory compliance.

• Complete MDS-related documentation and maintain accurate, compliant records
• Advocate for resident rights, dignity, and self-determination
• Coordinate discharge planning and community resources
• Collaborate with nursing, administration, and the interdisciplinary team
• Assist with survey readiness and regulatory compliance

What We're Looking For

• Certified, licensed, or registered in the state of practice (required)
• Bachelor's degree in Social Work or a related Human Services field
• At least 1 year of social work experience in a healthcare or geriatric setting
• Skilled Nursing Facility experience preferred
• Nursing experience in a SNF a plus
