Job summary

Blue Springs Wellness & Rehabilitation in Blue Springs, MO seeks a compassionate Social Services Designee to support residents and families with emotional, social, and psychological well-being in a skilled nursing setting.

You will conduct psychosocial assessments, assist with transitions, advocate for resident rights, and collaborate with the interdisciplinary team to ensure person-centered care.

Qualifications

• Certified, licensed, or registered in the state of practice (required).
• Bachelor’s degree in Social Work or a related Human Services field.
• Minimum of 1 year of social work experience in a healthcare or geriatric setting.
• Skilled Nursing Facility (SNF) experience preferred; nursing experience in a SNF is a plus.

Responsibilities

• Conduct psychosocial assessments and contribute to individualized care plans
• Support residents and families through transitions into long-term care and memory care
• Complete MDS-related documentation accurately and in compliance with regulations
• Advocate for resident rights, dignity, and self-determination
• Coordinate discharge planning and connect residents with community resources

• Assist with survey readiness and regulatory compliance efforts

Education
