POSITION TITLE: Social Services Coordinator

DEPARTMENT: Recuperative Care

REPORTS TO: Social Services Manager

LOCATION: Los Angeles, Ca

CLASSIFICATION: Non-Exempt

SALARY: $26.44 Hourly Rate

  • Work Shift: Sunday-Thursday, 11:30AM-8:00PM

Position Overview

The Social Services Coordinator provides post-hospital care services to guests experiencing homelessness who are transitioning out of an acute care hospital. The role involves applying professional experience and independent judgment to complex tasks, supporting organizational goals, performing administrative functions to assess needs, and serving as a community and organizational resource on healthcare and housing issues.

Responsibilities and Initiatives
  1. Complete Needs Assessments and Care Plans timely. Coordinate referrals for mental health, housing, etc.
  2. Facilitate discharge processes and locate housing for individuals. Complete discharge notes and communicate with community partners. Conduct warm handoffs when appropriate.
  3. Maintain up-to-date CRM database and partner contacts. Complete CES documents, HMIS, and utilize bed board applications for operational planning.
  4. Provide emergency service referrals and warm handoff referrals as needed.
  5. Attend care coordination meetings. Collaborate with nursing teams for weekly case conferencing.
  6. Practice harm reduction, support guest interventions such as de-escalation, crisis prevention, motivational interviewing, etc.
  7. Engage with guests through daily check-ins, building rapport and trust.
  8. Manage caseloads of at least 25 guests, with flexibility for more.
  9. Complete home visits when necessary to track progress or for Rapid Rehousing.
  10. Adhere to administrative hours for documentation. Maintain a professional image and dress code.
  11. Collaborate with external case managers to provide services. Triage and intervene in behavioral concerns.
  12. Follow all policies, procedures, and regulations to ensure compliance.
  13. Adapt to schedule changes as needed. Perform additional duties as assigned.
  14. Always adhere to NHF policies and procedures.
Minimum Requirements
  • Bachelor’s Degree required; Master’s in social work preferred.
  • Minimum three (3) years relevant experience in healthcare, hospital education, social services, or similar.
  • Effective communication skills, positive attitude, strong organizational skills.
  • Proficiency in MS Word, Excel, Outlook, PowerPoint, and typing skills.
  • Reliable transportation.
  • Bi-lingual Spanish preferred.
  • TB test, fingerprint live scan, and background check required.
Physical Requirements

Frequent standing, walking, sitting, bending, stooping, squatting, reaching, twisting, kneeling, and pushing/pulling. Ability to lift up to 50 pounds. Effective verbal and written communication skills. Ability to operate a computer and operate in noisy environments.
