Job summary

The Macoupin County Public Health Department is seeking a Social Services Coordinator to coordinate and deliver case management to individuals and families engaging with public health programs. You will assist clients in accessing mental health, substance use, housing, and other social determinant resources to improve wellbeing.

Responsibilities include developing service plans with clients, documenting activities, and working with interdisciplinary teams.

Qualifications

• High School Diploma or GED required.
• Bachelor's or MSW in social work or related field from an accredited institution.
• At least two years of case management/health services experience preferred.
• Working knowledge of community resources and referral processes.
• Basic understanding of social work principles and client engagement.
• Ability to work independently and in a team; strong organizational and communication skills.
• Proficiency in EMR systems.
• BLS certification required or ability to obtain.

Responsibilities

• Coordinate and monitor services for clients to ensure timely access to resources.
• Develop and maintain individualized service plans with clients and teams.
• Provide referrals to housing, employment, healthcare, and social supports.
• Maintain documentation and required data; ensure HIPAA compliance.
