Job summary

North Bay Regional Center is seeking a Social Services Case Manager to coordinate services for individuals with developmental disabilities. The role emphasizes person-centered planning, ensuring IPC goals are pursued and appropriate resources are used before center funds are allocated.

Ideal candidates hold a relevant bachelor’s degree; a master’s degree is preferred, with at least three years of related experience and a bilingual Spanish-English ability.

Qualifications

• Bachelor’s Degree in a field offering content related to developmental disabilities (Social Work, Psychology, Counseling, Special Education, and/or Nursing).
• Master’s Degree preferred.
• 3 years’ experience working with individuals with developmental disability.
• Bilingual language ability (Spanish to English) is preferred.

Responsibilities

• Using person-centered planning to assist clients in using public and private resources before using regional center funds to purchase services.
• Monitor vendor services and verify services are delivered per purchase of service authorizations.
• Evaluate the effectiveness of clients’ Individual Program Plans and ensure community resources are available.
• Conduct annual reassessments of clients’ developmental status with the interdisciplinary team.
• Conduct client assessments on an occasional basis.
