Qualifications

• High school diploma or GED required.
• At least 2 years of experience in human services or related nonprofit work.
• Bachelor’s degree and/or certified community health worker (CHW) experience preferred.
• Combination of experience and education may be considered.

Responsibilities

• Assist callers navigating complex social service systems.
• Serve as triage point to connect callers with trusted community partners.
• Monitor referrals and track outcomes using I&R technology.

Education
