Job summary

PA Health Management is seeking a CHC Service Coordinator in Allegheny County, PA. The position requires a bachelor’s degree in a relevant field, (3+) years in service coordination preferred, and a valid driver’s license for regional travel.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Minimum of three years in service coordination or case management preferred.
• Experience with waiver-funded or I/DD populations preferred.
• Valid driver’s license and reliable transportation for regional travel.

Responsibilities

• Coordinate services with participants, providers, and agencies.
• Maintain participant-centered planning, service coordination, and documentation.
• Monitor outcomes and ensure regulatory compliance.
• Engage in face-to-face visits and routine calls as required.
• Participate in meetings, trainings, and interdisciplinary coordination.
