Job summary

Responsibilities

• Assist the Social Services Director with day-to-day activities within the building.
• Apply professional social work standards in a long-term care setting.
• Coordinate care and services with residents, families, staff, and interdisciplinary teams.
• Advocate for residents' rights and ensure dignity in all interactions.
• Participate in quality improvement and emergency/safety procedures.
• Respond to behavioral and psychosocial needs with assessments and referrals.
• Maintain social services caseload as assigned and collaborate with Behavior Management.
