Qualifications

• High School diploma or equivalent required.
• One year of experience in social work required.
• Degree in Social Work or related field preferred.

Responsibilities

• Applies standards of professional social work practice in long-term care.
• Develops and maintains social work policies and procedures reflecting current standards and regulations.
• Coordinates care with residents, families, staff and interdisciplinary team members to implement plans of care.
• Advocates for residents’ rights and ensures dignity and individuality.
• Responds to behavioral and psychiatric issues with assessments and referrals to appropriate providers.
• Maintains social services caseload as assigned.
• Collaborates with the facility’s Behavior Management Program to meet psychosocial needs.

Education

High School diploma or equivalent

Degree in Social Work or related field (preferred)
