Pruitthealth Corporation is seeking a Social Services Director I to coordinate and direct Social Services in accordance with federal, state, and local regulations, and under the direction of the Administrator.

The role requires ensuring residents’ rights are protected, coordinating psychosocial assessments, discharge planning, and involvement with family councils, while maintaining high standards of care in a long-term care setting.

Qualifications

• Bachelor’s degree in Social work or related field required.
• Minimum of one year of supervised social work experience in healthcare or long-term care.
• MSW preferred.

Responsibilities

• Apply standards of professional Social Service work practice.
• Respect the rights, dignity and individuality of each resident in all interactions.
• Advocate for residents’ rights and coordinate psychosocial support as needed.
• Respond to behavioral/psychiatric issues with assessments and referrals.
• Coordinate discharge plans with residents and families.
• Assist with advance directives in accordance with Federal regulations.
• Address grievances and follow up to resolution.
