Job summary

Southern HealthCare Management is seeking a Social Services Assistant for our long-term care and short-term rehabilitation center in Lake Worth, TX. You will welcome and admit residents, complete psychosocial assessments, and help develop care and discharge plans to ensure a positive resident experience while supporting treatment goals.

The role works closely with the Social Services Manager on a Monday–Friday dayshift, providing compassionate support and coordinating services to meet residents'

Qualifications

• Minimum high school diploma or equivalent.
• Bachelor's degree in social work or a human services field.
• One year supervised social work experience in a health care setting.
• One or more years' experience in health care education/ marketing / insurance preferred.
• Familiar with medical terminology.
• Proficient in PC use.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments, care plans, and discharge plans.
• Support the social service function in a long-term care and rehabilitation nursing center.
• Work closely with the Social Services Manager to ensure a positive resident experience.

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday -- Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
