Job summary

Southern HealthCare Management is seeking a Social Services Assistant to support the social service function in our long-term care and short-term rehabilitation nursing center. You will welcome and admit new residents, complete psychosocial assessments, develop care and discharge plans, and help ensure a comfortable, positive experience while facilitating treatment plans.

The role follows a Monday–Friday dayshift schedule with opportunities to impact patient care and outcomes.

Qualifications

• Bachelor’s degree in social work or a related human services field is required.
• Minimum of one year supervised social work experience in a health care setting.
• Experience in health care education/marketing/insurance is preferred.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments, care plans, and discharge plans.
• Ensure a comfortable and positive experience while completing treatment plans.

You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
