Job summary

Northdale Rehabilitation Center in Tampa, FL is seeking a Social Services Assistant to support the social service function in a long-term care and short-term rehabilitation setting. You will welcome and admit new residents, complete psychosocial assessments, care plans, and discharge plans, and help ensure a positive patient experience while contributing to treatment plans.

The role follows a Monday–Friday dayshift schedule; prior experience in healthcare social work is valued.

Qualifications

• Minimum high school diploma or equivalent required.
• Bachelor's degree in social work or a bachelor's degree in a human services field including sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years' experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.

You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday - Friday, dayshift work schedule.
