Job summary

Fundamental is seeking a Social Services Assistant to support discharge planning under the supervision of the Social Services Director. The position offers a posted wage range of $17.00 to $18.00 per hour in South Carolina, with comprehensive benefits for full-time staff.

The role involves performing initial assessments, guiding families on Medicare/Medicaid options, and coordinating with Case Management to ensure smooth discharges.

Qualifications

• Must have a High School Diploma or equivalent (G.E.D).
• Social Work License preferred, not required.
• Must have experience in long term care or skilled nursing facility.
• Reliable transportation required.

• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident.

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
