Fundamental is seeking a Social Worker with dementia certification to work with discharge planners, families, and care teams to coordinate tours and follow-up procedures at our Granger, IN facility.

The ideal candidate has 3+ years in long-term care, excellent communication, and strong organizational skills, with reliable transportation and the ability to prioritize tasks in a fast-paced environment. Compensation ranges from $30.00 to $33.00 per hour, with a comprehensive benefits package.

• Skilled communicator with strong interpersonal skills.
• Reliable transportation required.

Responsibilities

• Assist in the initial contact as necessary with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patients/residents
• Keep abreast of current federal and state regulations as well as professional standards

Position Summary

Posted Salary Range

USD $30.00 - $33.00 /Hr.

Wage Commensuration

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions, including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
