Qualifications

• Bachelor’s degree or higher (Social Work preferred).

We are seeking a compassionate and motivated Social Services Assistant to join our team and play a vital role in supporting our residents and their families. If you’re passionate about making a meaningful impact in long-term care, we’d love to hear from you.

Shift:

• Monday-Friday

• 9:00 AM-5:30 PM

Key Responsibilities:

• Develop and implement a comprehensive social services program for residents and families

• Provide counseling and address medical-related social needs

• Coordinate and facilitate family meetings and ongoing communication between residents, families, and the care team

• Advocate for residents’ rights and serve as a liaison among all stakeholders

• Manage discharge planning and transitions of care

• Deliver exceptional service in a supportive and respectful environment
