Job summary

Brookshire Rehabilitation and Healthcare Center in Huntsville, AL is seeking a Social Services Assistant to support residents and families through advocacy and coordinated programs. You will contribute to care planning and help residents transition at discharge, working with a compassionate, patient-centered team.

The role offers $17–$20 per hour, full-time daytime shifts, and a benefits package including insurance, 401(k), PTO from day one, and ongoing professional development opportunities.

Qualifications

• Experience in social services in healthcare or long-term care.
• Understanding of resident rights and care planning processes.
• Excellent communication and interpersonal skills.
• Ability to work with residents, families, and interdisciplinary teams.
• Associate’s or Bachelor’s degree in Social Work or related field preferred.

Responsibilities

• Assist in coordinating and implementing social services programs for residents.
• Support residents and families in adjusting to facility life and addressing concerns.
• Help develop and maintain care plans that align with resident needs and preferences.
• Facilitate resident and family meetings to provide guidance and assistance.
• Provide resources for financial, emotional, and legal support as needed.
• Assist with discharge planning and ensure a smooth transition for residents returning home or to another care setting.
• Maintain accurate documentation and reports in compliance with facility policies and regulations.
