Aston Group is hiring a Social Services Assistant for our Skilled Nursing Facility. You will support the Director of Social Services, assist in planning and delivering resident-centered social services, and help coordinate with an interdisciplinary team to improve residents’ quality of life.

The role emphasizes counseling, advocacy, and documentation of care plans, with a focus on regulatory compliance and compassionate resident support. Strong interpersonal skills are essential.

Qualifications

• Bachelor's Degree in Social Work preferred; Bachelor’s Degree in Human Services or related field will be considered.
• Previous experience in social services required.
• Prior long‑term care or skilled nursing experience strongly preferred.
• Must possess any certifications or licensure required by state regulations.

Responsibilities

• Assist in planning and implementing a comprehensive Social Services program that provides counseling, support, and advocacy for residents and their legal representatives.
• Identify medical-related social needs of residents and provide appropriate services to meet both individual and collective needs.

Social Services Assistant

We are seeking a compassionate and dedicated Social Services Assistant to join our Skilled Nursing Facility team. In this role, you will support the Director of Social Services in delivering high-quality, resident-centered care that enhances each resident’s quality of life and overall well-being.

The Social Services Assistant plays a key role in planning, developing, organizing, implementing, and evaluating social services programs that support residents and their families throughout the length of stay.
