Job summary

Anoka Rehabilitation & Living Center, part of Volunteers of America National Services, seeks a Social Services Assistant to support the Social Service Director in fostering the physical, mental and psycho-social well-being of residents.

Responsibilities include coordinating transfers, completing essential assessments for planning and documentation, developing care plans, and participating in interdisciplinary conferences to ensure high-quality care outcomes.

Qualifications

• High school diploma or equivalent education required; Bachelor’s degree in Social Work or related degree preferred.
• Must be eighteen (18) years old or older at start date of employment.
• Ability to safely perform the essential functions of the job.
• Ability to work as part of a team.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

• Completes assessments of residents for Care Planning, MDS, CAAS, PPS and other required documents.
• Develops Care Plans and attends Interdisciplinary Care Conferences.

About the job:

The Social Services Assistant role is to support the Social Service Director in the care and treatment of residents entrusted to the facility and to attain or maintain the highest practicable physical, mental and psycho-social well-being of each resident.

Essentials:

• Coordinates resident transfers and discharges.
