Job summary

Centennial Post Acute in Anchorage, AK, is seeking a Social Services Assistant to support residents’ psychosocial needs, discharge planning, and access to community resources. You will document care plans and collaborate with the care team to ensure patient-centered support.

Ideal candidates bring experience in social services within healthcare, compassionate leadership, and strong organizational skills to help residents and families navigate illness and transitions while maintaining compliance

Qualifications

Responsibilities

• Develop, implement, and maintain social services policies and procedures.
• Assess residents' psychosocial and social service needs and support adjustment to illness or discharge planning.
• Provide guidance on Medicare/Medicaid and community resources; coordinate referrals with agencies and providers.
• Maintain accurate social service documentation, progress notes, and required reports.
• Participate in care planning meetings and interdisciplinary team Collaboration.
