The Avenue at Aurora is a premier post-hospital rehabilitation and skilled nursing facility committed to compassionate, person-centered care. We seek a Social Services Designee to support residents’ emotional and social adjustment, advocate for rights and quality of life, and coordinate care planning within an interdisciplinary team.

Ideal candidates meet Ohio requirements for the role, bring experience in long-term care, and demonstrate strong communication and organizational skills.

Qualifications

• Meets Ohio requirements for Social Services Designee.
• Long-term care/SNF experience preferred.
• Strong communication and organizational skills.
• Compassionate, professional, and resident-focused.
• Ability to work effectively with residents, families, and interdisciplinary teams.
• Experience with resident rights, care planning, or discharge planning is a plus.

Responsibilities

• Support residents with their emotional and social adjustment to long-term care and rehabilitation.
• Advocate for resident rights, dignity, preferences, and quality of life.
• Complete psychosocial assessments and care plans.
• Participate in care conferences and interdisciplinary team meetings.
• Assist residents and families with concerns, transitions, and discharge planning.
• Coordinate community and social service referrals.
• Maintain accurate and timely documentation.
• Assist with admissions and provide support to residents and families.
• Participate in quality improvement initiatives and staff education.
