The Sanctuary at the Woodlands in Cullman, AL is seeking a compassionate Social Worker to join our dedicated care team. You will assess patient needs, develop care plans with an interdisciplinary group, and provide counseling to patients and families to support well-being.

Qualifications

• Social Work License valid in Alabama.

Responsibilities

• Conduct patient assessments to identify social service needs.
• Develop and implement personalized care plans with interdisciplinary team.

• Advocate for patient needs within the facility and coordinate with external resources.
• Maintain accurate and confidential patient records.
• Oversee discharge planning and continuity of care post-discharge.
