Job summary

RegalCare at Harwich is seeking a compassionate, experienced Director of Social Services to oversee all psychosocial care for residents and their families, promoting dignity and quality of life.

You will coordinate social services across the facility, conduct psychosocial assessments, develop care plans, guide discharge planning, ensure regulatory compliance, maintain accurate records, mentor staff, and collaborate closely with the care team to support each resident’s goals.

Qualifications

• Bachelor's Degree in Social Work or related field; MSW preferred.
• Minimum 2 years of experience in long-term care or healthcare.
• Familiarity with MDS 3.0 and care planning process.
• Strong communication, organizational, and interpersonal skills.
• Knowledge of federal and state regulations regarding social services in nursing facilities.
