Job summary

West Pico Terrace Healthcare & Wellness Center is seeking a Social Service Director to oversee residents’ psychosocial needs and coordinate care. Requires a Bachelor's in Social Work or human services and at least one year of supervised healthcare experience; strong communication, interpersonal, and organizational skills are essential.

Responsibilities include orientation, discharge planning, care plan updates, records, family communication, QMPI program, and coordinating resident finances;

Qualifications

Responsibilities

• Ensure residents’ psychosocial and concrete needs are identified and met per federal, state and company requirements.
• Orient residents to the center.
• Discharge planning.
• Implement and update Resident Care Plan and Social History.
• Maintain records of outside referrals.
• Assist in coordinating residents’ financial affairs.
• Communicate needs and plan of care to resident, families, responsible parties and staff.
• Participate in meetings, in-services and training as required.
• Meet with consultants as required.
• Organize and facilitate Family Council (participate as requested).
• Implement and maintain QMPI program.
• Assist with coordination of resident room moves.
• Presents professional image to consumers through dress, behavior and speech.
• Adheres to company standards for resolving consumer concerns.
• Ensures that all consumer/resident rights are protected.
• Organize and maintain a professional work setting.
